Under the Paperwork Burden Reduction Act, Oakridge Public Schools is no longer required to mail Forms 1095-C. Oakridge Public Schools will provide them upon request. Employees may request Form 1095-C by emailing the request to schan@oakridgepublicschools.org or mailing the request to 275 S Wolf Lake Rd, Muskegon, MI 49442. Employees may call 231-788-7107 with questions or concerns.
