Register Students for School Year and Complete Forms

ALL parents of Early Childhood Special Education and K-12 students (not GSRP) need to complete the Final Forms registration process.  This includes new/returning students.  If your contact information has not changed, parents must still sign several forms and permission slips annually.  For the safety of children, parents must annually complete technology use forms, immunization forms, transportation forms, and health conditions forms.  If you have relocated within the district, a new proof of residency is still required.

Need Help Logging In to Final Forms?

We have made all forms paperless to improve efficiency.  Please go to https://oakridge-mi.finalforms.com/ or visit our Enroll/Register webpage to register your child(ren) and complete all the required forms.

School Bus Transportation Requests

Transportation staff will be making phone calls to parents of bus riders to notify them of pickup/drop off times the week of August 19.
